Leslie Augustus

That from on or about May 1, 2006, to on or about October 31, 2006, or on one or more dates therein, in the vicinity of 3510 S. Michigan Avenue, at Police Department Headquarters, a Supervisor and/or Sergeant gave SDEO Leslie Augustus an order and/or directive to report to work, and SDEO Leslie Augustus failed to report to work, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

That from on or about May 1, 2006, through on or about October 31, 2006, or on one or more dates therein, SDEO Leslie Augustus was inattentive to duty in that SDEO Leslie Augustus failed to report to duty without having any medical and/or vacation and/or compensatory time, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

That from on or about May 1, 2006, through on or about October 31, 2006, or on one or more dates therein, SDEO Leslie Augustus failed to follow Department directives in that SDEO Leslie Augustus failed to call in sick to her unit of assignment an hour before her scheduled reporting time and/or within 15 minutes of her scheduled reporting time, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

That from on or about May 1, 2006, to on or about October 31, 2006, or on one or more dates therein, SDEO Leslie Augustus failed to report to her tour of duty after requesting and/or being denied time, thereby impeding the Department's efforts to achieve its policy and goals or bringing discredit upon the Department.

From in or around 2002, to the date the charges were filed, or for some period of time
therein, the Respondent fraternized and/or maintained an association with Michael Meyers, an individual convicted of one or more felonies and/or misdemeanors.

Rule 10: SDEO Leslie Augustus, Employee Number 38619, is charged with violating Rule 10, "Inattention to duty," in the following instance:
Count I: That from on or about May 1, 2006, through on or about October 31, 2006, or on one or more dates therein, SDEO Leslie Augustus was inattentive to duty in that SDEO Leslie Augustus failed to report to duty without having any medical and/or vacation and/or compensatory time.

That from on or about May 1, 2006, through on or about September 30,
2006, or on one or more dates therein, SDEO Leslie Augustus, failed to
report promptly any anticipated absence from duty in that SDEO Leslie
Augustus failed to call in sick to her unit of assignment an hour before her scheduled reporting time and/or within 15 minutes of her scheduled
reporting time

That from on or about May 1, 2006, through on or about October 31, 2006, or on one or more dates therein, SDEO Leslie Augustus was absent from duty without proper authorization in that SDEO Leslie Augustus failed to report to her tour of duty after requesting and/or being denied time.
